Mobile Health Clinic

In partnership with Connect Health + Wellness, Laurel Park Middle School is the first Henry County Public School site to offer an on-site mobile health clinic for its students and staff.

Beginning Thursday, January 22, 2026, the Connect Health + Wellness Mobile Unit will be on campus every Thursday from 7:00 a.m. to 12:00 p.m. A medical provider and clinical staff will be available to provide convenient on-site services such as sick visits, well child checks, immunizations, sports physicals, etc. This service is designed to support families who may have difficulty visiting a regular doctor’s office during the school day.

Important information:

  • CHW welcomes uninsured and insured patients, including private insurance and Medicaid
  • A Sliding Fee Discount Program (based on household income) is available for families without insurance.
  • This service is optional. No student will be seen without parent/guardian consent.
  • Paper copies of the consent form are available upon request through the school front desk.
     

Participation in this program is completely optional for families. However, if families decide to participate, to prepare for your student(s) to use this service if the need arises, please take a moment to complete the attached consent form. Again, this service is optional; you do not have to complete this form.
